My big toe hurts; could it be gout?

Gout is a complex type of arthritis characterized by the excess production of urate crystals. Urate crystals form because of high levels of uric acid in your blood. Without treatment, these crystals can build up in the joints (such as the big toe joint), causing inflammation and pain.

Humans naturally make urate crystals by breaking down purines –– naturally occurring substances. Your body typically breaks down purines, but if you eat a poor diet or drink lots of alcohol, they can build up in your bloodstream and trigger gout.

If you have intense joint pain in your big toe, knee, or wrist, seeking treatment is essential.

What are the symptoms of gout?

Gout symptoms include the following:

  • Intense joint pain
  • Limited range of motion
  • Inflammation and redness
  • Lasting discomfort

Often, gout flares occur suddenly and without warning. Symptoms are typically worse during the first 4-12 hours.

How does a podiatrist diagnose gout?

Your provider reviews your medical records, asks about your symptoms, and completes a podiatry exam. They look at your feet and toes, checking for redness and swelling, and order several lab tests, including:

Blood work

Blood work measures the amount of uric acid in your blood. High levels of uric acid increase the risk of gout but don’t guarantee it.

Joint fluid test

During a joint fluid test, your podiatrist looks at a sample of your joint fluid under a microscope, checking for uric acid crystals.

X-rays

X-rays provide photos of your joints, allowing your provider to rule out other potential causes of inflammation.

If your provider confirms you have gout, they make personalized treatment recommendations.

How is gout treated?

Gout treatment usually includes a combination of prescription medication and lifestyle changes. A&A Foot and Ankle Specialists, LLC might recommend:

  • Non-steroidal anti-inflammatory drugs (NSAIDs)
  • Colchicine, a prescription anti-inflammatory drug for gout
  • Corticosteroids to control inflammation and pain
  • Medications that block uric acid production
  • Medications that help your body eliminate uric acid
  • Limiting alcoholic beverages
  • Exercising regularly
  • Maintaining a healthy weight

Your podiatrist might also recommend dietary changes, such as eating more fruits and vegetables and avoiding red meat.
